What is the Crisis Resolution Home Treatment Team?

The Team is made up of two distinct components:-

The Crisis Team undertakes assessment at Royal Glamorgan /St. Tydfil’s Hospital for people who are experiencing a mental health crisis. The Service is accessed via telephone and is open 24hrs a day, 365 days a year.

The Home Treatment Team provides intense home support for people in a mental health crisis. This is undertaken at the person’s home, between the hours of 8am to 8pm.

Who can refer to the Crisis Team?

We will accept a referral from a variety of sources. We will accept referrals from Service Users, their carers, General Practitioners, Police, Accident & Emergency, Mental Health Day Services and the voluntary sector.

I have a Care Coordinator; can I have an appointment with the Crisis Team?

Yes, but only out of office hours. When someone is in a crisis, it is usually most appropriate that they speak with someone they know. So whilst we will offer an appointment outside of office hours, during office hours we would ask that you contact your care coordinator or deputy care coordinator.

What will happen when I phone the Crisis Team?

When you phone us, you will speak to a qualified mental health practitioner. Initially, a brief discussion will take place to determine how best we can help you. We do not always offer an appointment, sometimes we will signpost people towards the most appropriate support. Whatever the outcome, we will attempt to ensure you receive the appropriate support and help

What can I expect from a Crisis Assessment?

The first thing that we expect of you is to attend the Royal Glamorgan Hospital (South) or St Tydfil’s Hospital (North). When you have arrived you will be asked to take a seat in the waiting room so that a Doctor is contacted. You will then be invited into the Crisis Suite to discuss your concerns with a doctor and a crisis practitioner. This constitutes an assessment.

Following the Assessment, a number of outcomes can be expected.

Examples of what might happen are: -

  1. Advice and Support
  2. Sign posting to appropriate services
  3. Referral to secondary services
  4. Admission to Acute Services

Can I receive Home Treatment?

Home treatment is available to anyone who is considered to be in “secondary care services”. By this we mean, those people who receive support from the CMHT, Consultant Psychiatrist, Inpatient, Assertive Outreach Team, Primary Care Liaison, Doctors within the Mental Health Directorate undertaking home visits and Emergency Duty Team. The referral is made by the person in secondary care services who know that person best.

If you are not in secondary care, then your first port of call is the Crisis Team.

Who is in the team?

The teams are made up of nurses, social workers, occupational therapists, support workers, a psychologist and a doctor.

What can I expect if I am referred to Home Treatment?

If you are referred to Home Treatment the first thing that will happen is that a meeting is convened between the person who has made the referral, a member of staff from the Home Treatment Team and you, to discuss how best to support you. A plan will be agreed upon by all parties, which identifies where you need support and what we will try to do to help.

Following this you will be visited by the Home Treatment Team. The person at the center of your support will be your care coordinatotor. There will be regular meetings between all parties so that we ensure that the support is right for you.
